                             2.  Signature to be written, NOT PRINTED.  Use first full name, middle initial and last name.

 

 

 

Envelope preparation:

 

                (1)           Put stamp on envelope.

 

                (2)           Affix the label that matches the test date.

 

                (3)           Fill in return address on back.

 

                (4)           Mail as soon as possible.  Check deadlines for mailing.  Make sure you have enclosed check or money order, have placed a stamp on the envelope, and have filled in both front and back of the envelope.  Your admission ticket should arrive no later than one week before test date.
